What happens at an initial evaluation?
Initial evaluations are in person. Jane handles appointment booking and medical history and records collection.
How is eligibility determined?
Eligibility depends on a qualifying medical condition under Florida law and an individual physician evaluation. We will review your history and explain your options.
Can established patients renew by telehealth?
Eligible established patients may renew by telehealth with the physician who previously examined and certified them in person.
Are state card renewal and physician certification renewal the same?
No. State card renewal and physician certification renewal are separate. Jane will collect the medical history and records needed for your visit.
What happens if I transfer my certification?
Jane will collect your information so the physician can review your history and explain the next steps for transferring care.
